How to Book an appointment:
To request an appointment please email your referral to info@heartwise.me and we will contact you to arrange a suitable time.

If you are booked in for a cardiac test, a text message will be sent with a link for your consent. Please kindly fill in prior to appointment.
For your appointment, please bring:
• Your Medicare card and Health Care/Concession card (if applicable)
• Your referral form or letter from the doctor
• The Patient’s height and weight
Kindly wear comfortable two piece attire for your appointment.
Please provide at least 24 hours notice for any appointment changes, as late cancellations may incur a fee.
HeartWise is a private billing clinic and out of pocket fees apply. Please ask our reception team about fees and Medicare rebates.
